C -Card registration
The C-Card Scheme provides condoms and sexual health information for young people. Online registration is only available if you are aged 16-19* and live within our services area within Lincolnshire.
If you are under 16, you can register for a C-Card by visiting your nearest outlet that provides the registration service.
If you are 16 or over, you have the option to register online via the form below. We will then send you a C-Card bundle via post (in discreet packaging). Once you have your C-Card, you are then allowed to pick up free condoms at any participating outlet.
The information you provide on this form will be treated as highly confidential, and we will not pass it on to anyone else, unless we are legally obliged to do so.
*Up to 25 for inclusion of those with learning disabilities
